I want to purchase this theme (rtMedia = http://demo.rtcamp.com/rtmedia/?theme=InspireBook) but I need to know if it comes in three columns as well and if not how can that be done?

InspireBook theme is not in 3 columns layout. However, you want to make customization on it, you may consider to hire our team.

Or if you are technically sound you can create a child theme and do required changes.

Okay can I extend the registration form to add other elements such as gender, date of birth, etc?

Yes you can extend the registration form. You can use following plugin for extend registration form “Buddypress Xprofile Custom Fields Type”
